ANNUAL PROGRESS ASSESSMENT
Every PhD student past Year 1 must prepare an annual report of progress toward the degree; this is reviewed by the area Director and the Graduate Chair, and the Major Professor and Co-Director, as appropriate.

7 days before the Candidacy Exam (formerly called ATE or Specialty Exam) you need to print and have signed the:

After having returned the signed Eligibility form to 222 LOV, students taking the Candidacy Exam should print or pick up the:

PROSPECTUS DEFENSE (ACM area only)
Within one year of admission to Candidacy, the student should defend a prospectus of the intended dissertation research. The student should print or pick up the:

DISSERTATION DEFENSE
There are departmental and university forms. All of these (except the Outside Committee Member report) are to be returned to 222 LOV (Esther Diaguila). The dissertation copy should be distributed to committee members (4) weeks before the intended defense. At that time, print or pick up a copy of "Forms necessary for PhD Dissertation Defense".
7 days before the Doctoral Dissertation Defense, you need to print and have signed the:
